New patients are responsible for filling out paperwork prior to the scheduled appointment with Dr. Yadava. This paperwork is required to be completely filled out when you arrive to your appointment. You will need to arrive 15 minutes before the scheduled time to allow the front office to look over the paperwork and scan the paperwork, driver’s license and insurance card(s) in. If the you show up without this paperwork completed, your appointment will need to be rescheduled as Dr. Yadava runs on time.
